Penn Foster’s licensing and accrediting organizations require that each student have a signed Enrollment Form on file at the school. This form should have been sent to you for your signature. Students under eighteen years of age must also have this form signed by a guarantor (and a parent or guardian if this person is different than the guarantor). The signed Enrollment form can be mailed to the Student Service Center, 925 Oak Street, Scranton, PA, 18515 or faxed to 570-961-4030. If you are faxing this document please fax both sides
If you do not have the Enrollment Form please call Student Services at 888-427-1000 to request one mailed to you. You can also request that an Enrollment Form be mailed to you by sending Student Services an email. Log on to the student website and click “Contact Us” under the heading “What Do You Want To Do?”. You can logon by going to www.pennfoster.eduand clicking “Student Logon”.
Failure to return a signed Enrollment Form to the school will result in interruption of your studies.
